Fees & payment

$175  |  90 minute initial evaluation​

$125 - $140 | Individual & Family Therapy

I take payment directly from my clients for services provided, and I accept cash and personal checks.

​

I accept four insurances: BC/BS, Aetna, Cigna, and Medicare. I am also an "out-of-network" provider, which means that if your insurance company offers out-of-network benefits, you should receive some reimbursement for the cost of my services. Most insurance companies that provide out-of-network benefits cover between 50% - 75% of the cost per session, after their deductible is met. I routinely provide my clients with appropriate documentation to get reimbursement for my services.

The best way to find out what your out-of-network coverage includes is to contact your insurance company.

Questions to ask your insurance company about your mental health benefits:

• Do I have out-of-network mental health benefits?

• What amount will I be reimbursed for the following services with LCSW-C?

  • CPT (Current Procedural Terminology, or Procedure) Code 90791 (initial consultation) at a fee of $175.00.

  • CPT Code 90834 (subsequent psychotherapy sessions) at a fee of $125.00 per session.

  • CPT Code 90837 (individual therapy sessions) at a fee of $140.00 per session.

  • CPT Code 90847 (family therapy sessions) at a fee of $125.00 per session.

• Is there a deductible I need to meet each year before I can begin to receive reimbursement for sessions?

• Is there a session limit per year?

• Is there pre-authorization required? If so, what information do you need for this? Is there a specific form that needs to be filled out?
